INSURANCE

We accept Medicare, Medicaid, and Private Insurance as well as offer a sliding scale for uninsured.  We welcome new patients and look forward to providing your primary care services to the fullest. 

POPULATION HEALTH SERVICES

We offer population health services including Medicare Annual Wellness Visits and Chronic Care Management. 

Medicare Annual Wellness visits are available to all Medicare beneficiaries one time each year as a free service.  We have dedicated nurses who work with you and your provider to ensure you get the most out of your Medicare Annual Wellness Visit.

Chronic Care Management is a service for Medicare patients with two or more Chronic Conditions such as Hypertension, Diabetes, Osteoarthritis, and many more conditions that our patients must manage daily.  If you have Medicare and two or more Chronic Conditions you can qualify to have a Population Health Nurse assigned to contact you monthly to assist you in managing your conditions and ensure you are on track to quality chronic care management.
